Registration Information for Coquitlam Lions Water Polo


New athletes must submit their birth certificates. The number of new players admitted depends on the number of returning players.
 


Registration Form 2009

 All athletes must:

- fill up one registration form for each player in a family.

- provide parents' email address is required to received updates/schedules via email.


Code of Conduct Form

 Each player and parent/guardian must sign the league Code of Conduct Form.
 

Due to insurance concerns, no athlete can enter the water unless all fees and forms are complete.
